While the position is primarily remote, occasion visits to our hospice center may be required. Key Responsibilities\n

As a Licensed Clinical Social Worker, you will engage in a variety of tasks that include but are not limited to:

• Assessing, diagnosing, and evaluating patients’ mental health status using the Diagnostic and Statistical Manual of Mental Disorders and other tools, tailoring individualized care plans that consider cultural diversity and special needs.

• Managing and monitoring patients’ progress, employing evidence-based interventions designed to foster recovery and enhance quality of life.

• Documenting and maintaining treatment records and reports in alignment with regulatory standards and organizational protocols, ensuring accuracy and timeliness.

• Collaborating with patients and their families to develop comprehensive treatment plans that reflect their needs and desired outcomes, while advocating for their best interests.

• Coordinating referral services and linking patients to appropriate therapy providers and other necessary resources.
