Key Responsibilities\n

• Conduct thorough ongoing assessments to determine the mental health needs and goals of patients.

• Facilitate effective communication among supervisors, care staff, family members, and guardians to review and discuss individual progress in treatment.

• Develop and share a tentative schedule with supervisors that outlines appointments, home visits, and necessary observations to monitor patient advancement.
\n
• Organize and conduct home visits to fulfill requirements for observations, chart reviews, and interviews that guide treatment decisions.
\n
• Ensure that treatment plans are effectively implemented and adapted to meet the evolving needs of patients.
\n
• Participate in interdisciplinary team meetings to collaborate on patient care and share insights on treatment progress.
\n
• Document monthly and quarterly progress notes that accurately reflect updates in patient treatment and progress.
\n
• Provide ongoing support through follow-up interviews and home visits to ensure consistent care and adjustments to treatment as needed.
\n
• Maintain meticulous records to ensure information is accessible for staff and supervisors.
\n
• Act as a liaison with the broader community, including other mental health professionals and organizations, to support patient development goals.

• Develop and refine training materials to enhance staff competence in implementing treatment protocols effectively.

Patient Population / Client Focus\n

You will work with a diverse clientele that includes both adults and youth, facing a range of mental health challenges. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a relevant field.

• An active and unrestricted Professional Counselor (LPC) license in the state of Colorado.

• A minimum of two years’ experience in a counseling role, preferably in a clinical setting.

• A strong understanding of legal and ethical standards governing mental health practices.

• Excellent communication and interpersonal skills, capable of establishing rapport with a varied clientele.

• Demonstrated ability to work collaboratively with healthcare teams in a cooperative manner.

• Proficiency in electronic medical records (EMR) software and other related technologies.

Preferred Experience\n

• Specialization in family therapy, couples therapy, trauma-informed care, or substance use disorders is highly preferred.
