Develop and implement individualized treatment plans based on each resident’s plan of care and physician orders. Administer therapeutic exercises, manual therapy, gait training, and other prescribed interventions to restore function and reduce pain. Supervise, coordinate, and direct the work of Physical Therapist Assistants (PTAs). Explains procedures and treatments to gain residents’ cooperation and allay apprehension. Monitor and document resident progress, modifying treatment plans as needed to achieve optimal outcomes. Maintains residents’ medical records with thorough documentation of evaluations, treatments provided, progress notes, and response to therapy. Collaborate with the interdisciplinary team to ensure continuity of care and successful achievement of rehabilitation goals. Educate residents and their families on home exercise programs, safety precautions, and strategies to maintain progress after discharge. Recommend appropriate assistive devices, adaptive equipment, and durable medical equipment as needed. Observes emotional stability of residents, expresses interest in their progress, and prepares them for continuing care after discharge. Perform other duties as required to provide exceptional care to each resident. What You Need Active unencumbered licensure as a Physical Therapist (PT) in Virginia. Degree in Physical Therapy from an accredited program. Excellent communication and interpersonal skills. Highly organized and detail oriented. Ability to work independently or as part of a team.
