Work Environment\n

This position is entirely remote, providing you with the flexibility to work from home while still being an integral part of our healthcare team. You will interact with other professionals via telehealth platforms and electronic health records, ensuring coordinated patient care and effective communication. Our work hours are Monday through Friday, from 9:00 AM to 5:00 PM, allowing for a balanced work-life schedule.

Key Responsibilities\n

• Conduct thorough bio-psychosocial assessments for individuals with mental health challenges, substance abuse issues, and medical needs, tailoring assessments to accommodate diverse age groups and cultures.

• Provide individualized behavioral health counseling and support for individuals, couples, and families through intake evaluations, treatment planning, crisis intervention, and when necessary, referrals for substance abuse treatment.
\n
• Manage mental health records, including documenting evaluations and progress notes electronically. This entails maintaining accurate and timely records of each patient’s status and treatment plans.

• Serve as a liaison between patients and local resources, offering social work support and referrals to community services, such as medication assistance, to ensure holistic care.

• Collaborate with our Quality Improvement teams, actively participating in data collection processes to enhance service delivery and patient outcomes.

Patient Population / Client Focus\n

You will work primarily with patients in a hospice care setting, which means your focus will be on providing emotional and psychological support to individuals nearing the end of life. This requires sensitivity, adaptability, and a profound understanding of the unique challenges faced by these patients and their families. Your role will directly contribute to improving their quality of life, ensuring they feel heard, understood, and supported during this critical time.

Qualifications\n

• A valid and relevant LCSW license in the state of California is a prerequisite for this position.

• A Master’s degree in Social Work, complemented by at least three years of post-licensure professional experience in a clinical setting.

Compensation and Benefits\n
