Reliance Youth and Family Counseling Center is seeking a dedicated and compassionate Licensed Mental Health Therapist (LMFT, LPC, or LCSW) for a full-time role within our innovative integrated care team. This position, based in Gilbert, Arizona, will primarily operate in a remote setting, allowing you to make a meaningful impact from the comfort of your own workspace. Work Environment\n

This full-time therapist role offers a predominantly remote work environment, which promotes flexibility and work-life balance while allowing you to reach a diverse patient population. Core hours are Monday to Friday from 8:00 AM to 5:00 PM, with occasional evening shifts as required by patient needs. Key Responsibilities\n

• Conduct comprehensive mental health assessments that evaluate the interplay between medical conditions and mental well-being, ensuring individualized care plans.

• Develop, implement, and regularly review integrated treatment plans that address both psychiatric care and medical management.

• Facilitate both individual and group therapy sessions using evidence-based therapeutic approaches tailored to each patient’s unique needs.

• Coordinate closely with an interdisciplinary team of medical providers to ensure seamless and effective treatment for all patients.

• Provide crisis intervention services and immediate support during mental health emergencies, helping patients navigate acute distress.

• Educate and empower patients by teaching them about the significant link between mental and physical health, promoting active participation in their own care.

• Maintain accurate, confidential, and compliant documentation of patient interactions, treatments, and outcomes in accordance with legal and ethical standards.

• Engage in regular team meetings, case conferences, and collaborative care planning to share insights and strategies with colleagues.

• Stay informed about the latest research, best practices, and advancements in integrated mental health care.

Patient Population / Client Focus\n

This role will primarily focus on a diverse population experiencing co-occurring conditions. You will work with patients battling chronic pain, autoimmune disorders, substance use disorders, anxiety, depression, and other mental health challenges, all of which require intricate, multifaceted care strategies. The goal is to improve health outcomes for each patient through targeted interventions that bridge mental health and medical care.

Qualifications\n

• Master's or doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Current and valid licensure as an LCSW, LPC, or LMFT in the state of Arizona.

• Demonstrated ability to work with individuals coping with co-occurring medical and psychological conditions.

• Experience in delivering integrated care and efficiently collaborating with a multidisciplinary team of healthcare professionals.

• Strong interpersonal and communication skills, fostering a therapeutic alliance with patients and collaboration with colleagues.

• Proficiency in managing case files and using electronic health record (EHR) systems effectively.

Preferred Experience\n

While two years of experience as a licensed mental health professional is required, candidates with additional experience in integrated care settings and familiarity with evidence-based practices in mental health therapy will be favored. Experience in crisis intervention techniques and prior work with multi-disciplinary healthcare teams will enhance your candidacy.

Compensation and Benefits\n

The position offers a competitive salary range of USD 75,000 – 100,000 annually.
