Work Environment

As a Nurse Practitioner, you will work onsite within an Assisted Living Facility, where you will interact closely with residents and staff to provide essential health services. This environment is dynamic and community-focused, requiring adaptability and responsiveness to the varying health needs of residents. Your typical work hours will be from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. You may also participate in community outreach events or health fairs during weekends, further contributing to our mission of health promotion.

Key Responsibilities

• Conduct comprehensive health assessments and evaluate the medical history of residents, ensuring accuracy and thoroughness in diagnosis.
• Diagnose and manage both acute and chronic medical conditions, offering preventive care and promoting healthy lifestyle choices specific to the community’s demographics.
• Collaborate with community organizations and stakeholders to design and implement effective health education initiatives, engaging residents and their families in proactive health management.
• Provide culturally sensitive care that respects the diverse backgrounds of our patients, while advocating for their rights and access to essential healthcare services.
• Coordinate referrals and follow-up care plans to guarantee continuity in the care of residents and improvement of overall health outcomes.
• Maintain detailed and accurate medical records in compliance with regulatory standards, utilizing electronic health record systems efficiently.
• Actively participate in community outreach to promote health awareness, conducting health screenings and educational workshops during events.
• Continuous professional development through staying updated with community health trends, evidence-based practices, and relevant healthcare policies.

Patient Population / Client Focus

Your primary focus will be on serving the residents of the Assisted Living Facility, many of whom may be part of underserved populations. You will address a wide range of health conditions influenced by socioeconomic factors and provide them with the tools and knowledge necessary to manage their health proactively. This role directly influences community health outcomes, and your work will empower individuals to take charge of their health in meaningful ways.

Qualifications

• A current certification as a Nurse Practitioner.
• Active state nursing license to practice in Nevada.
• A Master's degree in nursing from an accredited program.
• A minimum of three years of clinical experience, preferably within community health or public health settings.
• Exceptional interpersonal and communication skills, with an aptitude for building relationships with diverse populations.
• A strong commitment to advocating for health equity and social justice, particularly for vulnerable communities.

Preferred Experience

• Experience in conducting health assessments and managing chronic diseases in geriatric populations.

Compensation and Benefits

We are pleased to offer a competitive salary range of USD 120,000 – 130,000 per year, commensurate with your experience and qualifications.
