Work Environment

This full-time position requires an onsite presence within a hospital environment, where you will engage directly with patients and families. The typical work schedule spans from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Participation in community outreach events or health fairs during weekends is occasionally expected. Key Responsibilities

• Conduct thorough health assessments and screenings for a diverse population, developing individualized care plans based on patient health histories and socioeconomic contexts.
• Diagnose and manage a spectrum of acute and chronic medical conditions, ensuring that you practice preventive care and promote health awareness among patients.
• Collaborate with community organizations, health agencies, and stakeholders to create health education programs designed to inform and empower the community.
• Advocate for patients’ rights and ensure equitable access to healthcare services, focusing on culturally sensitive practices.
• Coordinate referrals and follow-up care to maintain continuity of care, monitoring patient progress and adapting treatment plans as necessary.
• Document all patient interactions meticulously in compliance with regulatory standards, ensuring seamless communication within the care team.
• Engage actively in community outreach initiatives and health fairs to enhance community engagement and raise health awareness.
• Regularly update your knowledge of community health trends and evidence-based practices to inform patient care and program development.

Patient Population / Client Focus

The Nurse Practitioner will primarily serve patients from a diverse range of backgrounds in North Las Vegas, focusing on families and individuals facing barriers to healthcare access. Qualifications

• Current certification as a Nurse Practitioner and active nursing license in the state of Nevada.
• A Master’s degree in nursing from an accredited institution is required.
• A minimum of 3 years of clinical experience is essential, preferably in community health or public health settings.
• Strong interpersonal and communication skills, enabling effective interaction with a diverse patient population.

Compensation and Benefits

We offer a competitive salary range of $120,000 to $130,000 per year, commensurate with experience and qualifications.
