Work Environment

The work setting is onsite at our Assisted Living Facility in North Las Vegas, where you will interact closely with residents and their families in a collaborative atmosphere. Our environment promotes professional growth and teamwork, equipped with the necessary tools and resources to provide high-quality medical care. You will enjoy a structured work schedule, typically spanning 9:00 a.m. to 6:00 p.m., with occasional evening clinics and requirements for community outreach on weekends, bringing healthcare services directly to those in need.

Key Responsibilities

• Conduct thorough health assessments and screenings for residents, evaluating their physical and mental health needs.
• Diagnose and create treatment plans for acute and chronic medical conditions, offering preventive care and health promotion to improve patient well-being.
• Engage in collaborative practice with healthcare professionals and community organizations to develop and implement effective health education programs.
• Provide culturally competent care, ensuring each patient receives advocacy for their healthcare rights and services.
• Oversee referrals and follow-up care, facilitating seamless transitions between different levels of care for our patients.
• Maintain accurate and detailed medical records, adhering to regulatory standards and promoting best practices in patient documentation.
• Participate in various community outreach activities and health fairs to promote wellness and increase accessibility to healthcare services.
• Stay informed about emerging community health trends and relevant healthcare policies to continuously improve patient care.

Patient Population / Client Focus

Our primary focus is on elderly residents within the Assisted Living Facility who often have complex health issues requiring personalized care strategies. You will support patients grappling with chronic conditions while providing preventive education and resources for healthier living. By understanding the specific needs of this population, you will enable families to navigate healthcare systems more effectively.

Qualifications

• Current certification as a Family Nurse Practitioner.
• Active nursing license in the state of Nevada.
• Bachelor’s degree in Nursing from an accredited program; a Master’s degree in Nursing is preferred.
• Minimum of 3 years of clinical experience in a primary care, community health, or public health setting.
• Exceptional interpersonal and communication skills, capable of engaging and building rapport with diverse populations.
• A strong commitment to advocating for social justice and improving healthcare access for vulnerable communities.

Preferred Experience

• Experience in geriatric care and an understanding of the unique healthcare challenges faced by elderly populations.
• Familiarity with electronic health record (EHR) systems for patient management and documentation.

Compensation and Benefits

This position offers a competitive annual salary ranging from $120,000 to $130,000, commensurate with your experience and qualifications.
