Work Environment

This position is based in Las Vegas, Nevada, where the vibrant community and diverse population present unique healthcare opportunities. You will work onsite at an assisted living facility, bringing your skills directly to the individuals in need of care. The typical work schedule is designed to support community engagement, running from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Participation in community events or health fairs may also be required on weekends, providing an excellent opportunity to connect with the community directly.

Key Responsibilities

• Conduct thorough health assessments, including physical examinations, health histories, and screenings, to evaluate the health needs of individuals and families.
• Diagnose and manage both acute and chronic medical conditions, effectively developing personalized treatment plans that incorporate preventive care and comprehensive health promotion strategies.
• Engage in collaborative efforts with community organizations and stakeholders to design and execute impactful health education programs aimed at enhancing community well-being.
• Deliver culturally competent care while advocating for patients' rights and ensuring their access to necessary healthcare services.
• Coordinate referrals and manage follow-up care, striving to guarantee continuity of care and ensure patients receive appropriate treatment at various healthcare levels.
• Maintain detailed and accurate electronic medical records, ensuring compliance with all relevant regulatory standards and facilitating efficient care delivery.
• Actively participate in community outreach initiatives and health fairs, providing essential health screenings to promote awareness and engagement.
• Stay abreast of emerging community health trends, evidence-based practices, and relevant healthcare policy changes to inform your practice.

Qualifications

• A Master's degree in Nursing from an accredited nursing program is required.
• Current certification as a Nurse Practitioner is essential.
• An active Nursing license in the state of Nevada is required.
