Work Environment

At our outpatient facility, you will work in a collaborative team environment that promotes professional development and innovation. The work schedule typically includes hours from 9:00 a.m. to 6:00 p.m., with some evening clinics operating until 8:00 p.m. Participation in weekend community outreach activities or health fairs is encouraged, allowing you to engage directly with the community. The role requires a commitment to an average 40-hour workweek, where each day brings new challenges and opportunities to make a difference.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families to identify health issues and risks.
• Diagnose and manage a variety of acute and chronic medical conditions, promoting preventive care and encouraging patient education on healthy lifestyle choices.
• Collaborate with local community organizations and healthcare stakeholders to develop, implement, and evaluate health education initiatives tailored to the population served.
• Advocate for patients by providing culturally sensitive care that respects diversity and promotes equitable access to healthcare services.
• Coordinate referrals and follow-up care, ensuring that all patients receive comprehensive care and support throughout their health journey.
• Maintain accurate and up-to-date medical records, adhering to regulatory standards to facilitate continuity of care.
• Engage in community outreach efforts and health fairs, conducting health screenings and providing valuable resources to empower community members.
• Stay informed about emerging community health trends, evidence-based practices, and healthcare policies relevant to your role.

Patient Population / Client Focus

This position primarily serves the diverse residents of Las Vegas, many of whom face significant health disparities due to economic and social factors. You will work with individuals and families from various backgrounds, providing care that is not only clinical but also empathetic and culturally relevant. Your focus will be on educating patients about preventive health measures, chronic disease management, and access to community resources, ultimately enhancing their overall well-being.

Qualifications

• Current certification as a Nurse Practitioner and an active state nursing license.
• A Master’s degree in nursing from an accredited program.
• A minimum of 3 years of clinical experience as a Nurse Practitioner, preferably in community health or public health settings.
• Strong interpersonal and communication skills, with a proven ability to work effectively with diverse populations.
• A deep commitment to social justice, health equity, and advocacy for underserved communities.

Preferred Experience

• Experience in implementing and managing community health programs.
• Familiarity with electronic health record systems and data management.

Compensation and Benefits
