Key Responsibilities\n

• Conduct comprehensive mental health assessments that take into account patients' medical conditions, ensuring a holistic understanding of their needs.

• Develop personalized and integrated treatment plans that address both psychiatric and medical issues facing each patient. This process includes working closely with their primary care providers.

• Facilitate individual and group therapy sessions utilizing evidence-based practices such as Cognitive Behavioral Therapy (CBT) or Dialectical Behavior Therapy (DBT), tailored to the unique needs of patients.

• Provide crisis intervention services during mental health emergencies, ensuring immediate support and access to care as needed.

• Educate clients on the interplay between mental and physical health, enabling them to better manage their conditions.

• Maintain detailed and confidential records in line with HIPAA regulations and best practices for ethical documentation.

• Participate actively in team meetings and interdisciplinary care planning sessions to ensure coordinated approaches to patient care.

• Stay informed about current trends and research in mental health and integrated care to continuously enhance service effectiveness.

The role primarily serves individuals experiencing co-occurring conditions, including mental health issues such as anxiety, depression, and PTSD, along with chronic illnesses like fibromyalgia, autoimmune disorders, and substance use disorders. Qualifications\n

• A Master's or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Current and valid licensure as an LICSW, LMHC, or LMFT in Washington State.

• Proven proficiency in addressing the mental health needs of individuals with co-occurring medical conditions.

• Experience working in integrated care settings and collaborating with healthcare professionals is essential.
