Position Overview

Full Motion Physical Therapy is seeking a dedicated and skilled Licensed Physical Therapist to join our team based in Jacksonville, Florida. As a full-time therapist within our rehabilitation center, you will play a critical role in enhancing the quality of life for our patients through tailored physical therapy interventions. This position offers a competitive annual salary ranging from USD 100,000 to 120,000, commensurate with experience.

Key Responsibilities

• Direct and implement individualized physical therapy treatment plans aimed at preventing disability, relieving pain, and restoring muscle function aligned with each patient's capabilities.
• Oversee and mentor physical therapy assistants to ensure the proper execution of treatment plans and facilitate optimal patient outcomes.
• Conduct thorough evaluations of patients' functional abilities utilizing diagnostic testing to assess muscle, nerve, and joint functionalities.
• Assess home environments for rehabilitation feasibility and provide tailored recommendations to enhance clients' independence.
• Deliver physical therapy services in both clinic and home settings as per physician directives, maintaining a high standard of care.
• Administer therapeutic exercises, including manual techniques and instruction on non-manual exercises for improved patient functionality.
• Utilize additional modalities such as traction and massage therapy to facilitate the healing process.
• Fit and adjust prosthetic and orthotic devices, consulting with physicians as necessary to ensure patient comfort and functionality.
• Engage with the interdisciplinary team to develop, review, and update care plans while participating in case conferences.
• Document changes in patient conditions meticulously and communicate concerns or recommendations to ensure continuity of care.
• Prepare and maintain clinical documentation including evaluations, progress reports, and discharge summaries to meet regulatory standards.
• Guide patients and their families in understanding the physical therapy process and the proper usage of rehabilitation devices.
• Complete functional assessments and collaborate with physical therapy assistants to enhance patient care delivery and outcomes.
• Remain accessible to support and guide the Physical Therapy Assistants during patient treatment sessions.
• Execute additional responsibilities as assigned to further the company's mission and patient care objectives.

Patient Population / Client Focus

Our patient population includes a diverse range of individuals, from post-operative recovery clients to those managing chronic illnesses and mobility impairments. You will work with patients of all ages, tailoring your approach to meet the unique challenges they face and participating in their journey towards recovery and enhanced quality of life.

Qualifications

• Graduation from an accredited physical therapy program.
• A minimum of one year of experience in a physical therapy setting, preferably in acute care or home healthcare.
• Current and valid licensing as a Physical Therapist in the state of Florida.
• Current CPR certification.
• A valid driver's license and automobile insurance are required for home care services.
• Strong interpersonal communication skills, with a patient-focused approach to care.

Preferred Experience

• Experience working with diverse patient populations and conditions.
• Familiarity with electronic health record (EHR) systems.
• Certification in specialized therapy techniques is a plus.
