Work Environment

This position is onsite and involves visiting patients in their homes throughout Jacksonville, Florida. As a Physical Therapist in our organization, you will work Monday through Friday from 8 AM to 5 PM, allowing for a structured schedule that supports work-life balance. You will provide individualized treatment plans that align with each patient's personal goals, utilizing a wide array of therapeutic techniques in familiar settings.

Key Responsibilities

• Conduct comprehensive assessments to recognize and fulfill each patient's objectives and requirements.
• Maintain a manageable patient load of one individual per hour, ensuring thorough and dedicated attention to each case.
• Administer cost-effective therapy sessions that enhance patients' mobility, implementing non-invasive approaches to minimize reliance on medication.
• Develop and articulate comprehensive treatment plans through diverse therapeutic modalities, customizing fitness and wellness programs to fit individual patient needs.
• Document all services provided accurately and thoroughly, utilizing EMR software such as WebPT to ensure continuity of care.
• Monitor patient progress, evaluate outcomes, and adjust treatment plans appropriately, communicating effectively with patients and caregivers.
• Encourage and motivate patients throughout their treatment journey, focusing on optimizing their functional capabilities.
• Collaborate with other healthcare providers to enhance the overall care continuum and ensure multidisciplinary approaches to rehabilitation.
• Educate patients and families on exercises, self-care techniques, and preventive measures to enhance recovery and sustain mobility.

Patient Population / Client Focus

As a Physical Therapist at Radiance Physical Therapy, you will primarily support patients recovering from orthopedic injuries, surgeries, and chronic conditions affecting mobility. Our client population includes seniors, post-operative patients, and individuals seeking rehabilitation after sports injuries. Each patient's treatment program will be designed considering their unique circumstances, lifestyle, and goals, aiming to offer a personalized recovery experience.

Qualifications

• Master's degree in Physical Therapy (MPT) or Doctorate in Physical Therapy (DPT) is required.
• Current Florida state license to practice as a Physical Therapist with no record of board or disciplinary actions.
• Proficient in manual therapy techniques and strategies to promote movement and recovery.
• Strong skills in designing and implementing tailored exercise programs that address individual patient needs.
• Familiarity with Electronic Medical Records (EMR) systems, specifically WebPT, is preferred.

Preferred Experience

• Prior experience as a Physical Therapist in orthopedic outpatient settings is highly desirable.
• Demonstrated ability to work independently in a home healthcare environment.
• Experience working with special populations, including geriatrics and post-operative rehabilitation, is a plus.

Compensation and Benefits
