Work Environment

The FNP will work onsite at our outpatient office located in Toms River, a vibrant community that is home to diverse populations and rich cultural heritage. Your work schedule will generally be from 9:00 a.m. to 6:00 p.m. with occasional evening clinics extending until 8:00 p.m. You may also participate in community outreach events or health fairs on weekends, as we strive to reach all corners of our community, raising awareness about healthcare services and preventive measures.

Key Responsibilities

• Conduct thorough health assessments, screenings, and patient examinations to identify medical conditions and needs, ensuring personalized care plans.
• Diagnose and manage a wide range of acute and chronic medical conditions, implementing preventive health strategies to promote wellness.
• Collaborate with local organizations to develop and execute community health education programs that address prevalent health issues.
• Foster an inclusive environment by providing culturally sensitive care and advocating for the rights and access of all patients to necessary healthcare services.
• Manage follow-up care, ensuring effective coordination of referrals and communication with specialists to maintain continuity in patient care.
• Document and maintain comprehensive medical records in compliance with healthcare regulations and organizational protocols.
• Stay informed about the latest trends in community health and participate in ongoing education to enhance your practice.

This role focuses on a diverse client base, including families from various socioeconomic backgrounds, ethnicities, and health literacy levels. You will work closely with underserved populations, empowering them to take charge of their health through education, support, and actionable health interventions. Qualifications

• Current and valid Nurse Practitioner certification recognized in the state of New Jersey.
• Possession of an active New Jersey nursing license.
• A Master’s degree in Nursing from an accredited institution is required to meet the educational standards of this role.

Preferred Experience

• A minimum of 4 years of clinical experience in family medicine, community health, or similar settings is preferred, showcasing your ability to manage diverse patient needs.
• Experience working with vulnerable or underserved communities, demonstrating an understanding of the social determinants of health.
• Strong communication and interpersonal skills to effectively engage with diverse patient populations and collaborate with multi-disciplinary teams.

Compensation and Benefits

The position offers a competitive salary range of $120,000 to $220,000 per year, commensurate with your experience and qualifications.
