Key Responsibilities\n

• Provide supportive counseling services to patients in alignment with your clinical training and state licensure, focusing on their psychological and emotional needs.

• Conduct educational and goal-oriented counseling for patients navigating transplant processes, helping them understand and cope with their medical journeys.

• Assist patients in accessing community resources such as home health services and vocational rehabilitation, facilitating their treatment outcomes and overall well-being.

• Stay updated on local vocational and educational rehabilitation programs, providing referrals that empower patients to engage in employment or educational opportunities.

• Evaluate patient awareness regarding advance directives, facilitating discussions with healthcare teams and families to ensure patient wishes are understood and honored.

• Participate as an integral member of an interdisciplinary team, assessing the psychosocial factors influencing rehabilitation and treatment success for each patient.

• Engage in collaborative care planning to devise effective interventions tailored to meet each patient’s rehabilitation goals and enhance their quality of life.

• Utilize established social work theories and methods, quality of life measurement tools, and patient education programs to support the treatment planning process.

• Deliver interventions that support patients in adjusting to dialysis and improving their psychosocial status, leading to better health outcomes.

Qualifications\n
