Key Responsibilities\n

• Conduct comprehensive assessments of patients to discern their mental health needs and develop individualized treatment plans that align with best practices in hospice care.

• Provide psycho-social support through a range of therapeutic modalities, including Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT), tailored specifically for our hospice patients.

• Document and maintain accurate clinical records in compliance with regulatory standards, ensuring that all patient information is confidential and secure.

• Facilitate communication among the interdisciplinary treatment team to foster coordinated and cohesive care for each patient.

• Educate patients and their families about mental health challenges and available resources to empower informed decision-making and support throughout the hospice experience.

• Stay abreast of the latest research and trends in social work specifically related to hospice and palliative care, integrating relevant findings into daily practice.

• Manage a caseload effectively, ensuring each patient's treatment goals are met while adhering to ethical standards of practice.

Patient Population / Client Focus\n

This role primarily serves patients who are in the hospice care phase of their lives, focusing on providing emotional and psychological support for patients and families facing end-of-life issues. Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• A valid LCSW license in the state of Illinois is mandatory.

Compensation and Benefits\n

We offer a competitive compensation package that includes a salary between $80,000 and $100,000 per year.
