This full-time position offers a hybrid remote work environment, allowing you the flexibility to conduct counseling sessions both via video chat and in-person at our Hospice Center. Key Responsibilities\n

• Conduct thorough assessments of clients’ emotional and mental health needs through virtual meetings and face-to-face consultations at the hospice.

• Develop, implement, and regularly update individualized treatment plans that address clients’ specific goals, preferences, and circumstances.

• Provide ongoing individual counseling sessions using evidence-based therapeutic techniques aimed at alleviating emotional discomfort and enhancing resilience.

• Advocate for clients and guide them through the process of setting achievable personal goals related to coping with their condition.

• Collaborate with physicians, nurses, and other healthcare professionals to ensure a coordinated approach to the holistic care of our clients.

• Participate in case management meetings to review client progress and adapt treatment plans as necessary.

• Facilitate support groups and workshops to educate clients and families on coping mechanisms and available resources.

Your primary focus will be on clients receiving end-of-life care, offering support to both individuals navigating their own experiences and their families managing complex emotions. Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited program.

• Current and valid Licensure as a Clinical Social Worker (LCSW) without any disciplinary actions.

• Proven experience in clinical social work including individual assessment, diagnosis, and treatment planning.

Compensation and Benefits\n
