While the role is primarily remote, there will be occasional in-person meetings and team collaborations focused on patient care and outcomes at our hospice facility in Fresno, CA.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through direct interviews, observations, and reviewing medical histories to evaluate the emotional and social factors affecting patients and their families during hospice care.

• Offer compassionate counseling and practical support to patients and their families, facilitating discussions around adjustment issues, emotional distress, and navigating the complexities of end-of-life decisions.

• Collaborate closely with a multidisciplinary team that includes physicians, nurses, chaplains, and rehabilitation specialists to develop tailored treatment plans that address the unique psychosocial needs of each patient.

• Assist patients and families in accessing a wide range of community resources, including grief support groups, vocational rehabilitation, and housing assistance, to ensure a holistic approach to care.

• Provide educational workshops and resources to patients and their families to empower them in making informed decisions regarding post-rehabilitation services and community support.

• Participate in discharge planning by coordinating resources and follow-up care, ensuring patients transition smoothly from hospice services to community resources.

• Regularly document care activities, assessments, and interventions in compliance with organizational policies and relevant regulations to maintain the integrity of patient records.

The primary patient population includes individuals receiving hospice care due to terminal illnesses, with a diverse array of backgrounds and needs. You will engage with patients facing various stages of grief, transition, and life adjustments, ensuring that their emotional and social well-being is prioritized as part of their overall care plan.

Qualifications\n

Applicants must hold a valid Licensed Clinical Social Worker (LCSW) certification and possess a Bachelor’s Degree in Social Work or a related field. A minimum of one year of experience in a similar setting, particularly within hospice or palliative care, is essential.
