Conduent Physical Therapy is seeking a dedicated and experienced Full-Time Physical Therapist to join our team in Chantilly, Virginia. Work Environment

As a Physical Therapist in this role, you will work onsite and engage with patients directly in their homes. Key Responsibilities

• Conduct thorough examinations and evaluations to identify patient needs and functional limitations, developing tailored treatment plans to meet individual goals.
• Provide ongoing assessments and consult with healthcare providers to ensure a seamless continuum of care for each patient.
• Maintain productive, respectful interactions with patients of all ages and backgrounds, following our organizational customer service standards.
• Document patient evaluations, treatment interventions, and progress in compliance with all relevant laws, regulations, and professional standards.
• Collaborate effectively within an interdisciplinary treatment team, engaging in regular communication with referral sources and physicians to enhance patient outcomes.
• Lead community outreach efforts and participate in program development, helping to shape the future of our home health services.
• Supervise and mentor other staff members, such as Physical Therapy Assistants, aides, and students, fostering growth and development within the department.
• Attend staff meetings and continue personal professional development through ongoing education and training.

Patient Population / Client Focus

The ideal candidate will have experience working with diverse patient populations, including those recovering from orthopedic injuries, surgical interventions, and chronic conditions. You will be pivotal in assessing patients' unique environments, which will enable you to create effective, real-world therapeutic interventions that significantly enhance their rehabilitation and recovery journey.

Qualifications

To excel in this role, candidates must possess the following qualifications:

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A valid State License in Physical Therapy.
• BLS certification to ensure patient safety during therapy sessions.
• Strong written and verbal communication skills to effectively document and discuss patient care activities.
• Proficiency in computer systems and word processing applications to maintain accurate records and communication.

Preferred Experience

Ideal candidates will have at least two years of experience in physical therapy, particularly in home care or outpatient settings. Familiarity with electronic health record (EHR) systems will enhance your ability to contribute to patient management and documentation requirements efficiently.

Compensation and Benefits
