Work Environment

This role is based onsite at patients' homes throughout the Tampa area, where you will have the unique opportunity to work closely with your clients in their comfortable, familiar environments. Physical therapists frequently communicate with nursing staff and interdisciplinary teams to review treatment plans and ensure comprehensive care for each patient.

Key Responsibilities

• Direct and administer physical therapy treatments focused on preventing disability, alleviating pain, and restoring muscle function in accordance with individualized care plans.
• Oversee the implementation of care plans by physical therapy assistants, ensuring appropriate application and adherence to the prescribed treatments.
• Conduct thorough evaluations of client capabilities using diagnostic tests related to muscle, nerve, joint, and functional abilities.
• Assess the patients' home environments to recommend adaptations and modifications that enhance their rehabilitation potential and independence.
• Implement prescribed physical therapy interventions including manual exercises, traction, and therapeutic modalities to facilitate optimal recovery.
• Evaluate, fit, and adjust orthotic/prosthetic devices while providing pertinent recommendations to physicians for modifications based on client progress.
• Maintain detailed clinical records, including initial evaluations, progress notes, and discharge summaries, in compliance with regulatory guidelines.
• Actively participate in interdisciplinary team meetings and care planning to ensure optimal treatment outcomes through collaborative approaches.
• Provide instructional support and guidance to clients and their families regarding therapeutic exercises and usage of physical therapy tools.
• Ensure timely completion of documentation and readiness for audits, maintaining compliance with state and federal regulations.

Patient Population / Client Focus

Your primary focus will be on clients with orthopedic conditions requiring rehabilitation in a home setting. You will work with diverse patient demographics, from post-surgical orthopedics to chronic pain management cases, all of whom require tailored treatment approaches that respect their individual goals and home environments. It is essential to build rapport and trust with your clients, encouraging their active participation in their recovery journey.

Qualifications

• A Doctorate in Physical Therapy from an accredited program.
• Current, valid physical therapy licensure in Florida.
• Minimum of one year of clinical experience in an acute or home care setting.
• Current CPR certification and valid driver's license with reliable transportation.
• Proven ability to provide exceptional care while effectively communicating with patients, families, and team members.
• A strong passion for patient advocacy and commitment to excellence in clinical practice.

Preferred Experience

• Experience coordinating care in a home health setting.
• Proficient in using electronic health record systems.
• Background in working with a variety of orthopedic diagnoses.
• Previous roles that required leadership and mentorship skills, particularly overseeing assistants.

Compensation and Benefits

Prairie Lakes PT Services LLC offers a competitive annual salary between USD 100,000 and USD 120,000, commensurate with experience and qualifications.
