Position Overview

Greenfield Rehab and Wellness Center is actively seeking a full-time Physical Therapist to join our dynamic team. In this role, you will work onsite, primarily in home care settings, helping patients regain their mobility and enhance their overall quality of life. You will play a critical role in designing and implementing personalized rehabilitation programs that support our patients' recovery and functional independence.

Work Environment

As a member of our team, you will work in a collaborative and supportive environment alongside two experienced physical therapists and a physical therapy assistant (PTA). Our holistic approach ensures that each patient receives tailored treatment plans developed in alignment with their specific needs and health goals. Key Responsibilities

• Conduct thorough assessments to identify patients' physical objectives and tailor treatment plans accordingly.
• Manage a patient load efficiently, averaging one patient per hour to maximize personalized attention.
• Utilize a comprehensive range of therapeutic techniques to enhance patients' mobility and function.
• Develop and implement cost-effective therapy interventions that reduce the need for surgical options and promote natural recovery.
• Design and adapt individualized fitness and wellness programs to meet specific patient requirements.
• Document all patient interactions and treatment plans accurately in compliance with our EMR systems.
• Regularly evaluate patient progress and adjust treatment strategies to ensure optimal outcomes.
• Motivate and educate patients throughout their treatment journeys, helping them overcome challenges and achieve their personal health goals.
• Collaborate with a multidisciplinary team of healthcare professionals to provide comprehensive care to clients.
• Engage in continuous professional development by staying current with industry best practices and attending workshops when possible.

Patient Population / Client Focus

You will primarily work with individuals recovering from orthopedic conditions, including post-surgical rehabilitation and chronic pain management. Our clientele often consists of active adults engaging in therapeutic programs to regain their former capabilities, and your expertise will directly impact their recovery and quality of life.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited program.
• Active California Physical Therapist license with no disciplinary actions.
• Strong proficiency in manual therapy techniques and the ability to implement appropriate exercise regimens.
• Ability to efficiently utilize EMR software, specifically WebPT, to document treatment and progress.
• Strong interpersonal and communication skills to foster a positive therapeutic relationship with patients.

Preferred Experience

• Prior experience in orthopedic outpatient therapy settings is highly desirable.
• Experience in developing individualized rehabilitation programs and fitness/wellness plans.
• Strong background in cost-effective treatment modalities and patient education practices.

Compensation and Benefits

The compensation for this full-time position ranges from USD 100,000 to 120,000 annually, commensurate with experience and qualifications.
