Work Environment

In this role, you will work onsite, mainly at patients' residences throughout West Los Angeles. The position requires travel to various locations, emphasizing the importance of flexibility and adaptability in varied home environments. You will collaborate closely with patients, their families, and other healthcare professionals, ensuring a united approach to health and wellness.

Key Responsibilities

• Conduct detailed evaluations of patients' physical therapy needs, utilizing assessment tools and techniques to create individualized treatment plans that align with their specific goals.
• Implement evidence-based therapeutic interventions, closely monitoring patient progress through regular assessments and modifying treatment plans as necessary to ensure optimum patient outcomes.
• Educate patients and their families on proper exercise techniques tailored to enhance recovery, mobility, and overall health, thereby fostering independence and self-management of their conditions.
• Document detailed patient interactions and progress, maintaining accurate records in compliance with state and national regulations, while ensuring timely communication with referring physicians and other members of the healthcare team.
• Collaborate with interdisciplinary teams composed of physicians, nurses, and other therapists to coordinate comprehensive care plans that address the holistic needs of each patient.
• Uphold high standards of safety and quality by adhering strictly to infection control protocols, ensuring a safe environment for both staff and patients.
• Proactively educate patients on injury prevention strategies and self-care techniques to minimize the potential for relapse or recurrence of injuries.

Patient Population / Client Focus

Your focus will primarily be on adult and elderly populations dealing with a range of physical impairments, including post-surgical rehabilitation, chronic pain management, and mobility issues often associated with aging. You will tailor your approach to meet the unique needs of each individual, applying your skills to help them achieve their personal rehabilitation goals.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution is required.
• Current and valid state licensure to practice as a Physical Therapist in California.
• At least two years of experience in a clinical setting, preferably with experience in home healthcare.
• Strong interpersonal and communication skills, with an ability to connect compassionately with patients and their families.

Preferred Experience

While not mandatory, additional qualifications such as certifications in specialized therapy techniques, experience with telehealth services, or bilingual communication skills will be considered advantageous. Experience in geriatric care, orthopedics, or neurological rehabilitation is particularly valuable in our home care environment.

Compensation and Benefits

We offer a competitive salary range of $110,000 - $125,000 annually, commensurate with experience, along with a robust benefits package.
