Key Responsibilities\n

• Conduct comprehensive assessments to determine patient needs and implement appropriate care plans tailored to individual circumstances.

• Communicate effectively with supervisors, direct care team members, family, and other stakeholders regarding each client's progress and treatment modifications.

• Maintain a tentative schedule for appointments, home visits, and observations to ensure a well-organized workflow and monitoring of patient advancement.

• Coordinate necessary home visits to execute in-home observations, conduct chart reviews, and gather data insights for comprehensive patient assessments.

• Ensure that the treatment plan is effectively implemented and that necessary adjustments are made based on ongoing evaluations.

• Participate actively in interdisciplinary team meetings to collaborate on patient care strategies and share relevant updates.
\n
• Complete detailed monthly and quarterly progress notes documenting patient outcomes and treatment effectiveness.
\n
• Organize and deliver training sessions to ensure all staff members are proficient in the implementation of program plans and maintain updated documentation.

• Provide continued support through follow-up interviews, home visits, and observations to ensure holistic care for each patient.

• Act as a liaison between clients and the mental health community, advocating for support and resources to meet developmental goals.

• Assist in developing training materials and procedures aimed at enhancing program offerings for clients.

Patient Population / Client Focus\n

Your role will involve working with a broad spectrum of clients, including both youth and adults, who may be experiencing various mental health challenges. You will focus particularly on helping clients dealing with trauma, substance misuse issues, relationship dynamics, and family-related concerns. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field is required.

• Must possess active and unrestricted licensure as a Professional Counselor (LPC) in the state of Tennessee.

The salary range for this position is competitive, falling between USD 90,000 and 100,000 annually, commensurate with experience and caseload.
