Key Responsibilities\n

• Create and maintain a tentative scheduling framework that optimizes appointment logistics, home visits, and monitoring observations for clinical advancement.

• Plan and coordinate home visits for in-depth observations that include chart reviews and staff interviews, ensuring compliance with program protocols.

• Implement the established treatment plan and monitor its effectiveness, ensuring that modifications can be made as patient needs evolve.

• Participate actively in interdisciplinary team meetings to advocate for and discuss patient care plans with fellow professionals.

• Document monthly and quarterly progress notes, capturing vital information regarding patient development and outcomes.
