Key Responsibilities\n

• Provide individualized psychotherapy and mental health services to patients facing terminal illnesses, focusing on their emotional struggles and life transitions.

• Conduct comprehensive mental health assessments to evaluate the psychological well-being of patients and their families, ensuring that all aspects of their emotional, behavioral, and social functioning are considered.

• Develop and implement personalized, evidence-based treatment plans that respect the patient's cultural background and are designed to meet their specific needs.

• Utilize effective communication skills to establish trust and rapport with patients, families, and interdisciplinary teams while providing ongoing support and resources.

• Document all patient interactions, treatment plans, and progress notes accurately in our electronic health records system, ensuring compliance with privacy regulations and clinic policies.

• Participate in regular team meetings and case conferences, contributing insights on patient care and collaborating on multidisciplinary approaches to treatment.

Patient Population / Client Focus\n

The primary focus of this role includes working with patients in hospice care dealing with a range of emotional and psychological issues related to terminal illnesses, including but not limited to anxiety, depression, and grief. You will play a crucial role in providing support not only to the patients but also to their families, helping them navigate difficult conversations and emotions surrounding end-of-life issues.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current state licensure as a Licensed Clinical Social Worker (LCSW) in California.

• Proven clinical skills, including assessment and treatment planning, with an emphasis on providing patient-centered care in challenging settings.

Compensation and Benefits\n

The salary for this position ranges from $115,000 to $130,000 annually, commensurate with experience and qualifications.
