Work Environment

As a Home Health Physical Therapist, you will provide onsite care directly in patients' homes, allowing for a personalized approach to therapy that is both convenient and effective. Key Responsibilities

• Conduct thorough evaluations of patients' physical abilities, limitations, and rehabilitation needs in a home setting.
• Develop individualized treatment plans based on evidence-based practices and patient goals, utilizing various therapeutic modalities such as manual therapy, exercise regimens, and assistive device fittings.
• Implement treatment strategies that aim to enhance patients' strength, flexibility, and overall mobility.
• Document patient progress comprehensively, modifying treatment plans as progress dictates while maintaining regular communication with other healthcare team members.
• Provide education and training to patients, family members, and caregivers about home exercises and the use of assistive devices.
• Stay current with professional development by attending workshops, engaging in continuing education, and participating in relevant professional organizations.

Patient Population / Client Focus

In this position, you will work with a diverse range of patients recovering from surgical procedures, injuries related to falls or accidents, chronic conditions, and other functional impairments. Your role will involve not only physical rehabilitation but also emotional support, as you guide your patients through their recovery journey within the comfort of their own homes, catering to their unique backgrounds and abilities.

Qualifications

• Completion of a Doctorate in Physical Therapy (DPT) from an accredited institution.
• Valid Physical Therapy license in the State of Florida.
• Strong interpersonal and communication skills, both written and verbal.
• Proficiency in basic computer skills and documentation software.
• Adaptability and flexibility in adjusting to varying patient needs and environments.

Preferred Experience

While a minimum of two years of experience in a clinical setting is preferred, we welcome new graduates who are eager to learn and grow within our organization. Comprehensive mentorship and training will be provided to all new hires to ensure they are well-equipped for success in home health care.

Compensation and Benefits
