Key Responsibilities\n

• Provide compassionate counseling and support to patients and families dealing with mental health challenges, ensuring that each interaction fosters trust and understanding.

• Utilize tools and technology to identify patients at risk for readmission, implementing intervention strategies and maintaining thorough clinical documentation through our electronic medical record systems.

• Participate in essential transition conferences, collaborating with patients and their families to design personalized care plans that promote successful outcomes.

• Monitor individual patient progress and coordinate with interdisciplinary teams to share insights and adjustments to treatment plans as needed.

• Advocate for patients' rights and access to essential resources, ensuring that they are informed about their treatment options and available community services.

• Conduct thorough assessments and deliver psychoeducation to both patients and their families, enhancing their understanding of mental health and available support systems.

• Stay informed on government and private payer networks to assure appropriate transitions and continuity of care for our patients.

• Address the social and financial needs of patients by connecting them with vital resources and support services.
\n
• Maintain clinical competence and ethical decision-making standards while adhering to all mandated reporting laws relating to vulnerability and suspected abuse.

• Engage in continuous professional development and training opportunities to enhance your skills and knowledge.

Patient Population / Client Focus\n

As a Licensed Mental Health Therapist at HEARD, you will primarily serve a diverse patient population facing a mix of acute, chronic, and terminal illnesses. Your role will extend beyond individual therapy by providing essential support to families, understanding their unique challenges, and offering tailored guidance to help them navigate the mental health landscape effectively.

Qualifications\n

• A Master’s or Doctorate degree in Clinical Psychology, Counseling, or Clinical Social Work (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD) from an accredited program.

• An independent license in the state of Oregon (e.g., LCSW, LMFT, LPC).

• Demonstrated ability to manage a sizeable caseload efficiently while maintaining a high standard of patient care.

• A strong passion for making a positive impact in the lives of those dealing with behavioral health issues.

• Proficient in the efficient use of electronic medical records and communication technologies.

Compensation and Benefits\n

Competitive annual salary ranging from USD 50,000 to 80,000, commensurate with your experience and qualifications.
