Key Responsibilities\n

• Design, implement, and coordinate tailored support packages by assessing clients' varied needs and strengths in the context of their hospice care.

• Conduct in-depth interviews with clients and their families, documenting their emotional and practical circumstances, which helps inform personalized treatment plans.

• Collaborate with healthcare professionals to devise and monitor measurable goals, ensuring each client’s treatment plan is up-to-date and effective.

• Track and assess client progress through regular follow-ups, adjusting plans as necessary to address any emerging challenges or changes in their condition.

• Provide evidence-based counseling and psychoeducation to clients during sessions, equipping them with coping strategies and resources tailored to their unique situation.

• Maintain comprehensive and compliant client records, providing accurate reports on their status to share with team members and other relevant parties when necessary.

• Participate in case conferences and, when required, present case details in legal settings to advocate for the best interests of clients.

• Act as a key worker, effectively liaising with a diverse array of external community resources and services, ensuring clients are connected with appropriate care options as they navigate their illness.

The ideal candidate will work primarily with clients in hospice care, which includes individuals suffering from terminal illnesses and their families. Compensation and Benefits\n

The annual salary for this position ranges from $90,000 to $100,000, commensurate with experience.
