Key Responsibilities\n

• Conduct ongoing assessments to identify and address the diverse needs of the patients you serve, ensuring a personalized treatment approach.

• Maintain effective communication with supervisors, colleagues, and families regarding each patient’s progress, tailoring interventions as necessary to achieve therapeutic goals.

• Develop and present a tentative schedule of appointments, home visits, and observations to monitor and document patient progress in real-time.
\n
• Coordinate home visits to gather valuable insights for in-home observations, which may involve conducting chart reviews and staff interviews.
\n
• Ensure the efficacy of program implementation by closely monitoring adherence to treatment plans and recommending adjustments as needed.
\n
• Actively participate in Interdisciplinary Team Meetings, contributing to collaborative discussions aimed at optimizing patient outcomes.
\n
• Prepare and document monthly and quarterly progress notes, ensuring thorough records of patient interactions and treatment outcomes.
\n
• Organize and deliver training sessions for staff to enhance their capabilities in executing program plans and maintaining accurate documentation.
\n
• Facilitate follow-up interviews, home visits, and additional observations to provide ongoing support and resources for clients.

Patient Population / Client Focus\n

As a therapist at Advocate Aurora, you will work with a varied client base, including children, adolescents, adults, and families facing a spectrum of mental health conditions. This role encompasses a focus on issues such as trauma, substance use, and relationship challenges, allowing you to make meaningful impacts on the lives of those you serve.

Qualifications\n

• A Master’s or Doctorate degree in a relevant mental health field is required.

• Possession of active and unrestricted licensure as a Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), or Licensed Clinical Social Worker (LCSW) in California.

• A minimum of two years of clinical counseling experience, particularly in a clinical setting, is required.
\n
• Specialization in family therapy, couples therapy, substance misuse, or trauma counseling is highly desirable.

• A solid understanding of legal and ethical standards in mental health practice.

• Demonstrated ability to work collaboratively within a healthcare team.

Preferred Experience\n

While at least two years of counseling experience is required, candidates with specialized training or certifications in family systems therapy, trauma-informed care, or substance abuse treatment will be given preference.
