Work Environment

This position is onsite at our Rehabilitation Facility located in Sayreville, New Jersey. Key Responsibilities

• Conduct thorough evaluations of clients' physical abilities using diagnostic and prognostic assessments to create tailored treatment plans.
• Administer specialized physical therapy techniques, including manual therapy, therapeutic exercises, and modalities such as traction and massage, to alleviate pain and enhance mobility.
• Supervise Physical Therapy Assistants in their therapeutic interventions, ensuring compliance with the established care plan and state regulations while providing mentorship and guidance.
• Collaborate with clients, families, and the interdisciplinary team to monitor progress, adjust treatment plans as necessary, and educate on rehabilitation goals and strategies.
• Prepare and maintain clinical documentation, including evaluations, progress notes, and discharge summaries, ensuring accuracy and compliance with legal and organizational standards.
• Regularly assess the home environment to make recommendations that encourage client independence and facilitate safe home rehabilitation.
• Participate in case conferences and contribute to the broader team's efforts in holistic patient care strategies and outcomes.

Patient Population / Client Focus

We serve a diverse population of patients with various orthopedic conditions, including post-surgical recovery, injury rehabilitation, and chronic pain management. Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program; a Doctorate in Physical Therapy is preferred.
• Current licensure as a Physical Therapist in New Jersey, with no disciplinary actions on record.
• Valid CPR certification and a reliable driver's license with up-to-date automobile insurance.
• Strong communication skills with the ability to engage effectively with clients and their families.
• A passion for promoting patient education on rehabilitation and self-care.

Preferred Experience

While the minimum requirement is one year of experience in a physical therapy setting, candidates with experience in both acute care and home care contexts are preferred.
