The Physical Therapist will be working directly in patients' homes, allowing for a personalized and flexible therapeutic environment. Key Responsibilities

In this role, you will:

• Conduct comprehensive Physical Therapy evaluations for patients, particularly children, assessing their physical capabilities and needs.
• Develop, implement, and adjust individualized treatment plans based on the latest evidence-based practices, coordinating care with other service providers as necessary.
• Regularly re-evaluate and document clinical signs and symptoms to track patient progress, modifying treatment strategies as required to align with patient goals.
• Utilize the company's electronic health record system to complete all necessary documentation, including SOAP notes and progress summaries, promptly and accurately.
• Educate patients and their families about the recovery process, setting realistic expectations for rehabilitation outcomes.
• Achieve minimum utilization goals and participate in performance metrics to drive both patient success and business objectives.
• Collaborate with professionals from various disciplines within Ensemble Therapy to maximize patient care efficacy for those receiving multiple services.
• Embody and advocate for the company's core values in all interactions, serving as a representative of Vitality Care Clinic.

Patient Population / Client Focus

The ideal candidate will work with a diverse patient population, including children with developmental disorders, post-surgical patients, and adults or seniors experiencing mobility challenges. Your sensitivity to varying cultural backgrounds and the ability to communicate effectively with patients and families will be pivotal in providing effective treatment and fostering trust.

Qualifications

To be considered for this position, candidates must meet the following requirements:

• Hold a Bachelor's degree in Physical Therapy; a Master's degree or Doctorate in Physical Therapy is preferred.
• Be currently licensed in California to practice physical therapy with no restrictions imposed by the Board of Physical Therapy.
• Possess flexibility and adaptability, comfortable with both in-person evaluations and telehealth services where clinically appropriate.
• Have a genuine passion for working with a wide array of patients, embracing diversity in age, culture, and health backgrounds.
• Exhibit strong collaborative skills, fostering a team-oriented and service-focused environment.

Preferred Experience
