Key Responsibilities\n

• Conduct comprehensive assessments to determine the clinical needs and treatment goals of each client, utilizing evidence-based practices.

• Effectively communicate any changes in a patient’s clinical status to other healthcare team members and office staff promptly to ensure continuity of care.

• Educate patients and their families on medical social work interventions, treatment plans, and coping strategies tailored to their unique situations.

• Develop and implement care planning, transfer, and discharge functions, ensuring a coordinated approach to treatment.

• Proactively identify potential risks or threats to client health within their environments and implement appropriate interventions to minimize these risks.

• Assist patients and their families in addressing health-related financial, social, and emotional concerns, fostering effective coping mechanisms.

• Maintain accurate and detailed records to support treatment planning and evaluation processes, ensuring compliance with documentation requirements.

Qualifications\n

Compensation and Benefits\n
