Key Responsibilities\n

• Conduct thorough and ongoing assessments to evaluate the mental health needs of patients, ensuring that care is tailored to each individual.

• Facilitate open communication among the patient's support network, including family members, guardians, and other healthcare professionals, to monitor and discuss progress.

• Develop tentative schedules for appointments, home visits, and necessary observations, providing supervisors with updated plans for monitoring patient advancements.

• Coordinate home visits effectively to gather crucial observational data, conduct thorough chart reviews, and perform staff interviews as needed.

• Implement and ensure adherence to tailored program plans, making necessary adjustments based on ongoing patient feedback and therapy effectiveness.

• Participate actively in Interdisciplinary Team Meetings to contribute insights and collaborate on patient care strategies.

• Document monthly and quarterly progress notes, keeping precise records of patient development and achievements.

• Organize training sessions for staff and team members to enhance competencies regarding program implementation and documentation standards.

• Providing follow-up interviews and continual support through home visits to further the patient's mental health journey.

Compensation and Benefits\n
