Key Responsibilities

• Conduct comprehensive evaluations, which include reviewing patient histories, performing assessments, and documenting relevant findings and recommendations.
• Effectively communicate evaluation results and treatment plans to patients and their families, fostering a collaborative approach to goal setting that includes input from all involved.
• Skillfully implement treatment protocols based on individual evaluations, exhibiting mastery of techniques and approaches suited to patient needs.
• Work closely with interdisciplinary healthcare teams, recognizing and documenting necessary changes in a patient's condition to modify care plans as required.
• Execute thorough discharge planning and follow-up care processes, ensuring that care documentation is clear, concise, and timely, which includes detailed evaluation notes, daily encounter logs, and progress summaries.
• Exhibit strong time management skills to meet productivity expectations, including maintaining an up-to-date schedule and engaging in marketing efforts that promote clinic services.
• Contribute to departmental goals and secure compliance with all governing regulations to uphold our standards of care.

Patient Population / Client Focus

Qualifications

• Must possess a Bachelor's degree in Physical Therapy from an accredited institution.
• A high school diploma or GED is required.
• Active physical therapy license for the state of Florida is required.
• Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.

Preferred Experience

• A minimum of 1-2 years of experience in an outpatient physical therapy setting is preferred, with a proven track record of successful patient outcomes.
• Experience in home health care services is an advantage.
• Fluency in electronic health records (EHR) systems to streamline documentation and enhance patient care.

Compensation and Benefits
