Key Responsibilities Conduct thorough and ongoing assessments of clients to determine their mental health needs and develop individualized treatment plans. Facilitate open communication with supervisors, healthcare staff, and families to provide comprehensive updates on each patient’s progress and well-being. Generate a tentative schedule for appointments and home visits, ensuring consistent monitoring of client treatment goals and outcomes. Plan and coordinate home visits, including detailed chart reviews and staff interviews, to gather relevant data for ongoing patient evaluations. Oversee the effective implementation of personalized program plans, ensuring all interventions are aligned with best practices and tailored to each client’s needs. Engage in Interdisciplinary Team Meetings, providing insights and contributing to collaborative treatment strategies. Document monthly and quarterly progress notes that effectively communicate client developments and treatment adaptations. Organize and lead training sessions to ensure that all team members are proficient in program implementation and documentation processes. Provide extensive follow-up support through interviews, home visits, and observations to ensure continual progress toward client goals. Act as a liaison between the organization and the mental health community to facilitate resource sharing and public awareness. Assist in developing training materials and procedural manuals to enhance service delivery and program effectiveness. Patient Population / Client Focus The LPC will work with a diverse clientele, including both adults and youth, facing various mental health challenges. This role emphasizes a client-centered approach, fostering strong relationships based on trust and understanding. You will be instrumental in addressing needs related to family dynamics, substance misuse, trauma, and overall emotional well-being. Qualifications Minimum of a Master’s or Doctoral degree in Counseling, Psychology, or a related field. Active and unrestricted licensure as a Professional Counselor (LPC) in Oregon. A minimum of two years of counseling experience in a clinical or similar context. Demonstrated expertise in family therapy, couples therapy, trauma-informed care, or substance misuse treatment is highly preferred. A solid understanding of legal and ethical standards in mental health practice. Exceptional interpersonal and communication skills, with a proven ability to build rapport with clients from varied backgrounds. Experience working collaboratively within a multi-disciplinary healthcare team. Proficient with electronic medical records (EMR) systems and other relevant technology platforms. Preferred Experience Experience with evidence-based practices and a background in specialized approaches such as cognitive-behavioral therapy, dialectical behavior therapy, or motivational interviewing is highly desirable. Familiarity with trauma-informed care principles is advantageous in effectively serving our diverse patient population.
