Key Responsibilities\n

• Participate in interdisciplinary care team huddles to coordinate patient care and discuss psychosocial considerations impacting mental and physical health.

• Conduct thorough assessments and evaluations to develop individualized treatment plans targeting each client’s specific mental health needs.

• Deliver both individual and group therapy sessions utilizing evidence-based practices to promote emotional well-being and resilience.

• Foster a safe and supportive therapeutic environment where clients can explore challenges and work towards actionable goals.

• Collaboratively consult with primary care providers on psychosocial issues presented by clients during their medical appointments, ensuring a comprehensive approach to health.

• Evaluate and triage urgent and routine referrals from medical professionals, responding effectively to each patient's needs.

Patient Population / Client Focus\n

This role will engage with a diverse client population facing various mental health challenges, including depression, anxiety, relationship issues, and trauma. Qualifications\n

• A Master’s degree in Counseling, Social Work, Marriage and Family Therapy, or Mental Health Counseling is required.

• Must hold an active licensure as a Licensed Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Counselor (LPC) in the state of Pennsylvania.

• Strong organizational skills with the ability to manage multiple patient files, schedules, and treatment plans efficiently.

• Effective communication skills, both verbal and written, to clearly articulate treatment plans and progress to clients and other health care providers.
