Key Responsibilities

• Conduct comprehensive evaluations, screenings, and assessments of patients' physical capabilities, strengths, limitations, and needs, using standard physical therapy procedures and modalities.
• Develop individualized treatment plans aimed at addressing specific injuries, diseases, and disabilities, setting measurable goals that encourage patient progress.
• Assist patients and their families by providing education on physical therapy practices, exercise techniques, and injury prevention strategies.
• Maintain diligent documentation of patient interactions, treatment plans, progress updates, and relevant correspondence in compliance with regulatory and facility standards.
• Collaborate effectively with other healthcare professionals, including physicians, nurses, and caregivers, to ensure a cohesive and comprehensive approach to patient care.
• Lead training and supervisory roles for physical therapy assistants and interns, providing guidance in delivering high-quality care that adheres to our standards.
• Participate in quality improvement initiatives and departmental operational management, including scheduling, staffing, and resource allocation.

Patient Population / Client Focus

Qualifications

• A current state Physical Therapist license is essential for this role.
• A Bachelor's degree in Physical Therapy is required; a Master's degree or Doctor of Physical Therapy (DPT) from an accredited institution is preferred.
• At least two years of clinical experience in a healthcare setting, particularly in home care or outpatient therapy is desirable.
• A strong understanding of physical therapy interventions and modalities, as well as familiarization with electronic health record (EHR) systems, is necessary.
• Ability to perform tasks requiring physical strength and stamina, including lifting, standing, walking, and manual dexterity.

Preferred Experience

• Experience working with diverse populations and various age groups, able to adapt treatment approaches to meet individual needs.
• Familiarity with home health care operations and compliance regulations.
• Demonstrated ability to work collaboratively in a multidisciplinary team environment.

Compensation and Benefits
