Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through patient interviews, observational methods, and thorough medical record reviews to understand emotional and social challenges impacting recovery.

• Utilize evidence-based counseling techniques to provide emotional support to patients and their families, assisting them in coping with the complexities of adjustment and bereavement.

• Collaborate with a diverse rehabilitation team, including physical and occupational therapists, to develop and implement individualized treatment plans that address psychosocial aspects of care.

• Identify and assist patients in accessing vital community resources such as support groups, vocational rehabilitation services, and housing assistance, ensuring continuous support outside the hospice environment.

• Educate patients and families on the importance of community resources and post-rehabilitation services to foster a successful transition back to independent living.

• Involve yourself in discharge planning to ensure seamless transitions to home care or other care facilities, strengthening the support network around each patient.

Compensation and Benefits\n
