Work Environment

The position is an onsite role, primarily requiring travel to patients' homes throughout the Avenel area. Our therapists enjoy the flexibility of choosing their hours, working Monday through Friday. This autonomy allows them to balance personal commitments while providing high-quality medical care. Key Responsibilities

• As a Home Care Physical Therapist (PT), you will be responsible for evaluating and treating patients who are recovering from various conditions, including injuries, diseases, and surgeries, directly in their homes.
• You will design and implement customized treatment plans utilizing techniques such as therapeutic exercises, stretching protocols, hands-on therapy modalities, and specialized equipment to alleviate pain and enhance patient mobility.
• Maintain detailed documentation of patient treatments, response to interventions, and progress towards rehabilitation goals.
• Regularly reassess and adapt treatment goals, ensuring alignment with patient recovery progress, and update care plans as needed.
• Assist patients in selecting and fitting assistive devices, such as walkers or wheelchairs, to promote safer and more independent living.
• Educate patients, their families, and caregivers on physical therapy techniques and the correct usage of assistive equipment to support ongoing recovery efforts.
• Engage in continuous education and professional development activities to keep abreast of the latest treatments and therapeutic techniques, promoting best practices within the team.

Patient Population / Client Focus

Your role will focus on a diverse patient population, ranging from children to the elderly, each requiring personalized care tailored to their unique circumstances. By providing therapy in the home setting, you have the opportunity to build meaningful relationships and closely monitor the home environment's impact on recovery. You will have the tools to directly influence improved patient outcomes by delivering care at the most crucial time in their recovery.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A valid and active state license in Physical Therapy in New Jersey.
• Strong written and verbal communication skills essential for effective patient interactions.
• Proficiency in word processing and basic computer software, as well as electronic health record systems.
• Adaptability and flexibility to work in diverse and dynamic home environments.

Preferred Experience

• Prior experience in a physical therapy setting is preferred but not required; new graduates are encouraged to apply, with mentorship available for successful candidates.
• Experience with home health care or in-home physical therapy is a plus.

Compensation and Benefits
