You will primarily be engaged in providing services via telehealth, enabling you to communicate seamlessly with patients and their families. Additionally, you will coordinate with the hospice center facility to ensure that care is integrated and comprehensive. Your role will involve telephonic and virtual consultations, assessments, and therapy sessions, all aimed at delivering timely support and intervention.

Key Responsibilities\n

• Conduct thorough behavioral health psychosocial assessments to evaluate patients' needs and identify appropriate clinical interventions.

Qualifications\n

• A Master’s Degree in Social Work from an accredited graduate program.

• Must possess a valid California Board of Behavioral Sciences Licensed Clinical Social Worker (LCSW) certification.

Compensation and Benefits\n
