Key Responsibilities\n

• Conduct thorough assessments to identify patients' psychosocial needs, developing tailored treatment or referral plans that prioritize their well-being.

• Provide case management services, including assisting patients in navigating forms for entitlements and insurance, helping them achieve stability in their lives.

• Facilitate access to prescription assistance programs to support patients in mitigating medication costs, leveraging partnerships with pharmaceutical companies and local resources.

• Advocate for patients during interactions with community agencies, ensuring they receive the services and resources they require for successful reintegration into community life.

• Maintain meticulous and updated patient records, prepare documentation for treatment plans, and contribute to clinical reports necessary for ongoing evaluation.

• Coordinate transportation assistance for patients when needed to ensure they can access critical services, contributing to their overall treatment success.

• Participate in clinical and team meetings, actively contributing to case conferences to ensure a holistic approach to patient care.

Patient Population / Client Focus\n

As a Licensed Mental Health Therapist, you will work primarily with adults facing a broad spectrum of mental health challenges, including but not limited to anxiety, depression, trauma, and substance use disorders. Your role is crucial in connecting patients with therapeutic interventions and community resources that empower them towards recovery and improved quality of life.

Qualifications\n

• A Master’s degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling is required.

• Possession of an active and independent license as a Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).

• A minimum of two (2) years of relevant professional experience in a mental health setting is essential for this position.

Compensation and Benefits\n
