Full-Time Home Care Physical Therapist

Apex Physical Therapy is seeking a dedicated and compassionate Physical Therapist (PT) to join our team in Portsmouth, Virginia. Work Environment

The work setting for this position is predominantly onsite in the homes of our patients throughout the Portsmouth area. You will have variable working hours, including 40 hours weekly with a requirement for weekend availability as needed. As you manage your caseload, you will enjoy the flexibility that comes with home care, allowing you to tailor your approach to each patient's unique needs.

Key Responsibilities

• Assess, examine, and evaluate patients with disabilities or functional limitations to determine appropriate treatment plans that enhance quality of life.
• Create and administer effective treatment and discharge plans based on comprehensive evaluations and patient input.
• Conduct consultations with healthcare providers to ensure a coordinated continuum of care.
• Adhere to high customer service standards by effectively interacting with patients of all ages and cultural backgrounds.
• Maintain detailed documentation of patient treatment, evaluations, charges, and compliance with all legal and professional regulations.
• Communicate and collaborate with interdisciplinary treatment teams, physicians, and referral sources to ensure optimal patient care.
• Supervise and mentor assigned support staff while participating in community outreach and quality improvement initiatives.
• Engage in ongoing professional development through in-services and continuing education opportunities.

Patient Population / Client Focus

Your focus will be on providing compassionate care to a varied patient population in their homes, which may include the elderly, post-operative patients, those recovering from injuries, or individuals with chronic pain conditions. Each patient's holistic well-being will be at the forefront of your treatment approach, ensuring they receive personalized care tailored to their unique circumstances.

Qualifications

• A Bachelor's Degree in Physical Therapy is required for this role.
• Possession of a valid State License in Physical Therapy, with current BLS certification.
• Strong written and verbal communication skills, along with proficiency in word processing and computer skills.
• Ability to effectively collaborate within an interdisciplinary team framework while managing continuous patient interactions.

Preferred Experience

• Experience in home health care is a plus, particularly in developing patient-centered treatment plans in individual family environments.
• Previous work with patient populations dealing with orthopedic injuries or surgical recovery will enhance your application.
• Leadership experience in mentoring support staff or contributing to quality improvement efforts is advantageous.

Compensation and Benefits

The compensation for this role ranges from USD 100,000 to 120,000 annually, commensurate with experience and qualifications.
