Critical Rehabilitation Physical Therapy is seeking a dedicated and skilled Physical Therapist to join our team on a full-time basis. Work Environment

This Physical Therapist position is onsite, where you will primarily be working in the homes of patients. Key Responsibilities

As a Physical Therapist at Critical Rehabilitation, you will:

• Perform comprehensive evaluations, reviewing patients' histories, conducting assessments, and documenting findings to tailor individualized treatment plans.
• Effectively communicate evaluation results and treatment plans to patients and their families, actively involving them in goal setting to ensure their engagement in the recovery process.
• Implement evidence-based treatment protocols specific to each patient's unique circumstances and continually assess their progress towards recovery.
• Collaborate with other healthcare professionals involved in the patients' care, adapting treatment plans as necessary while documenting significant changes in their condition.
• Manage patient discharge planning effectively, ensuring that follow-up care and future rehabilitation needs are clearly communicated.
• Maintain accurate and timely records of patient care activities, utilizing electronic health record systems to create a coherent narrative of the patient's treatment journey.
• Manage your schedule effectively to meet productivity goals while participating in community outreach initiatives to promote enhanced access to care.
• Engage in professional development and continuing education to enhance your skills and contribute to organizational learning.

Patient Population / Client Focus

Our patients range from those recovering from surgeries to individuals with chronic conditions requiring ongoing rehabilitation. You will work closely with a diverse population, tailoring your techniques and treatments to meet the specific needs of each client, ensuring they receive the highest quality of care while recovering in the comfort of their homes.

Qualifications

The successful candidate will possess:

• A Bachelor's degree in Physical Therapy from an accredited institution.
• A current state license to practice as a Physical Therapist in Virginia.
• Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.

Preferred Experience

While a minimum of two years of clinical experience is required, candidates with extensive hands-on experience in home healthcare settings or specializing in geriatric rehabilitation are preferred. Experience with electronic documentation systems and interdisciplinary collaboration is also advantageous.

Compensation and Benefits
