Work Environment

The work setting is primarily onsite, within patients' homes, which allows for personalized, one-on-one care tailored to individual circumstances. You will collaborate with a dynamic team of healthcare professionals, including nursing staff, occupational therapists, and speech-language pathologists, to deliver comprehensive patient care. The schedule typically involves Monday to Friday hours, with rotating weekend hospital shifts as necessary, reflecting our commitment to flexibility and responsiveness in patient care delivery.

Key Responsibilities

• Conduct thorough evaluations of patients to identify impairments, functional limitations, and disabilities.
• Develop, implement, and modify treatment plans based on patient needs and physician orders, involving therapeutic interventions and activities.
• Maintain accurate and timely patient records including treatment notes, discharge summaries, and progress reports in accordance with healthcare regulations.
• Supervise and provide guidance to Physical Therapy Assistants (PTAs) and support personnel in delivering effective patient care.
• Collaborate with interdisciplinary teams to discuss patient progress and adapt intervention strategies as needed.
• Evaluate the effectiveness of treatment interventions and adjust plans as necessary to ensure optimal outcomes.
• Engage in professional development activities, including continuous education to stay current with best practices in physical therapy.

Patient Population / Client Focus

The position addresses a diverse patient population primarily comprising individuals recovering from injuries, surgeries, or chronic illnesses. Our patients range across various age groups, requiring tailored treatment plans that enhance their functional independence and promote recovery. You will focus on delivering care that respects their dignity and encourages their active participation in rehabilitation processes.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited university.
• Current state licensure as a Physical Therapist in Virginia.
• Strong written and verbal communication skills essential for interacting with patients and interdisciplinary team members.
• Proficient in the use of electronic health record (EHR) systems and general computer applications.

Preferred Experience

• A minimum of two years of experience in an outpatient physical therapy or home care setting is preferred.
• Experience in evaluating and treating post-operative patients and individuals with specific neuromuscular conditions.
• Demonstrated leadership skills in supervising junior staff and collaborating with various healthcare professionals.

Compensation and Benefits

Heartfelt Life Insights Center offers a competitive salary ranging from USD 80,000 to 100,000 annually, commensurate with experience.
