Work Environment

This position is set within a dynamic home care facility in Clearwater, FL, where you will visit patients in their homes, adapting treatment plans to fit their unique environments and needs. Key Responsibilities

• Conduct comprehensive evaluations of patients through thorough history reviews, physical assessments, and diagnostic measures to establish a personalized treatment plan.
• Engage patients and their families in discussing evaluation findings and treatment goals, ensuring they feel informed and involved in their care.
• Implement evidence-based treatment protocols tailored to patient presentations and evolving healthcare standards, utilizing a variety of therapeutic techniques and modalities.
• Monitor and document changes in patients' conditions, adjusting care plans as necessary and consulting with interdisciplinary healthcare teams to achieve optimal outcomes.
• Oversee patient discharge planning and follow-up care, providing necessary resources and recommendations for continued recovery.
• Maintain accurate, timely documentation of patient interactions, including initial evaluations, daily progress notes, and discharge summaries, while adhering to ethical and regulatory standards.
• Participate in departmental goals and objectives, contributing insights and support for marketing efforts that enhance community access to our services.
• Act as a clinical mentor for students and new team members, instilling best practices and fostering a culture of learning.
• Proactively pursue ongoing professional development and continuing education opportunities to maintain licensure and enhance clinical skills.

Patient Population / Client Focus

Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution.

• Current American Heart Association BLS for Healthcare Providers CPR certification.

Compensation and Benefits
