Work Environment

The position is offered onsite within our home care department, providing you with the opportunity to work closely with patients in their own environments. This approach allows for a holistic understanding of each patient's needs, enhancing rehabilitation outcomes. You will work Monday through Friday, from 8:00 am to 5:00 pm, allowing you to establish a consistent routine that benefits both you and your patients.

Key Responsibilities

• Adhere to state practice acts and relevant healthcare laws that govern the practice of physical therapy, including appropriate supervision protocols.
• Conduct detailed examinations and evaluations to create individualized treatment plans for patients with functional limitations and disabilities.
• Perform comprehensive patient assessments, integrating historical health information and utilizing standardized evaluation tools to determine treatment efficacy and document results precisely.
• Implement and adjust treatment protocols based on patient evaluations, responses, and ongoing healthcare developments to achieve optimal therapeutic outcomes.
• Collaborate with multidisciplinary healthcare teams to ensure cohesive care strategies and document any changes in patients' conditions or treatment needs.
• Manage accurate discharge planning and follow-up care, ensuring that all patient interactions are well-documented and compliant with evolving healthcare regulations.
• Participate in team meetings to contribute insights regarding patient access barriers and engagement with quality improvement initiatives.
• Maintain all corporate and clinical training standards, staying current with best practices and compliance requirements as mandated by state and federal regulations.

Patient Population / Client Focus

Your focus will primarily be on outpatient orthopedic patients who are medically referred for physical rehabilitation. You will provide one-on-one therapy sessions that cater to patients of all demographics, including post-operative recovery, athletic injury rehabilitation, and chronic pain management. Your role is pivotal in enhancing patients' mobility and overall quality of life through tailored interventions.

Qualifications

• A Bachelor's degree in Physical Therapy or a Doctor of Physical Therapy (DPT) from an accredited program.
• A valid physical therapy license in the state of Florida.
• Current CPR certification to ensure the safety and preparedness in emergency situations.

Preferred Experience

• At least two years of experience in an outpatient orthopedic setting, demonstrating proven skills in patient management and therapeutic interventions.

Compensation and Benefits
