Work Environment

This position is primarily onsite, necessitating travel to patients' homes across the Roseville area. You will thrive in a dynamic environment, interacting not only with patients but also with healthcare colleagues, including physicians, occupational therapists, and nursing staff. Key Responsibilities

• Conduct comprehensive evaluations of patients, including reviewing medical histories and performing physical assessments to determine suitable treatment plans.
• Design and implement individualized, evidence-based treatment plans reflective of each patient's goals, existing physical capabilities, and anticipated recovery timelines.
• Utilize a variety of physical therapy interventions, such as manual therapy techniques, exercise prescriptions, and educational resources, to optimize patient outcomes.
• Provide ongoing education regarding therapeutic exercises, posture techniques, body mechanics, and injury prevention strategies aimed at facilitating an active recovery process.
• Document therapy sessions, track patient progress meticulously, and adjust treatment plans as necessary, ensuring compliance with clinic protocols and regulatory requirements.
• Collaborate closely with multidisciplinary healthcare teams to provide a cohesive and well-coordinated care experience for patients.
• Maintain the highest standards of clinical care while adhering to all safety protocols and regulatory guidelines.

Patient Population / Client Focus

This role primarily serves adult patients needing rehabilitation support for a range of conditions, including but not limited to post-surgical recovery, stroke rehabilitation, chronic pain management, and mobility impairments. You will have the opportunity to develop meaningful relationships with your patients, offering personalized care that addresses their unique needs and empowers them towards independence.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program.
• An active and unrestricted Physical Therapist license in the state of California is required.
• Thorough knowledge of human anatomy and physiology applicable to physical therapy practices.
• Demonstrated ability to communicate effectively with patients, their families, and team members to enhance patient care.
• Capability to work both independently and collaboratively in a team setting, adapting your approach to meet a variety of patient and program needs.

Preferred Experience

• A minimum of 2 years of clinical experience as a Physical Therapist, ideally in a home care environment.
• Certification or specialized knowledge in fields such as orthopedics, geriatrics, sports medicine, or neurorehabilitation is highly desirable.
• Previous experience employing electronic medical record (EMR) systems to document patient care and track progress.

Compensation and Benefits

We offer a competitive annual salary ranging from $110,000 to $125,000, based on experience and qualifications. In addition to salary, our comprehensive benefits package includes:
