Work Environment

This role is performed onsite at patients' homes, allowing for a personalized approach to rehabilitation. As a part of your daily duties, you will manage your own schedule, performing evaluations and treatments in a variety of local settings. Your autonomy in the field will enable you to build meaningful patient relationships and provide care that aligns with each individual's unique needs and lifestyles. Expect to work Monday through Friday from 9 AM to 5 PM. While you will have independence in managing your caseload, you will also be supported by a collaborative team of healthcare professionals.

Key Responsibilities

• Conduct comprehensive assessments of patients' physical health by evaluating their medical histories, current physical abilities, and limitations through detailed patient interviews and physical examinations.
• Design and implement individualized rehabilitation plans that are tailored to each patient's specific needs, goals, and progress, using evidence-based practices to guide treatment strategies.
• Administer a variety of physical therapy techniques, including manual therapy, therapeutic exercises, electrotherapy, and modalities that address pain reduction and mobility improvement.
• Educate patients and their families about exercise regimens, body mechanics, ergonomic principles, and injury prevention, thereby empowering them to play an active role in their recovery journey.
• Continuously monitor progress throughout therapy sessions, making necessary adjustments to treatment plans to optimize patient outcomes while ensuring accurate documentation and record-keeping.
• Collaborate effectively with physicians, occupational therapists, and other healthcare professionals to ensure a unified approach to patient care, thereby enhancing service delivery.
• Foster supportive relationships with patients to build trust and motivation, helping them stay committed to their rehabilitation process.

Patient Population / Client Focus

In this role, you will work with a diverse patient population, including individuals recovering from surgery, those with chronic pain, sports-related injuries, or developmental challenges. Your ability to understand and adapt to each patient's unique circumstances will be crucial for providing high-quality care and promoting effective recovery.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• Possession of a valid state license to practice as a Physical Therapist or eligibility for licensure in California.
• Strong foundational knowledge of human anatomy, physiology, and common physical therapy practices.
• Exceptional communication skills and the ability to build rapport with patients, families, and multidisciplinary team members.
• A commitment to maintaining patient confidentiality and adhering to ethical best practices in healthcare.

Preferred Experience

• At least 2 years of experience as a practicing Physical Therapist; experience in pediatric care or acute care settings is a plus.
• Familiarity with adaptive equipment and modalities used in home care environments.
• Experience with electronic health record (EHR) systems for patient management and documentation.

Compensation and Benefits

The position offers a competitive salary range of $110,000 to $125,000 annually, alongside a comprehensive benefits package that includes:
