Your Role: Home Health Physical Therapist You will provide one-on-one care in patients’ homes, helping them regain strength, mobility, and confidence in their daily lives. Key Responsibilities Conduct in-home evaluations, start of care, and follow-up visits Develop and implement individualized treatment plans Improve strength, balance, mobility, and functional independence Educate patients and caregivers on safety and equipment use Document visits using WellSky Kinnser EMR Collaborate with physicians, RNs, and the care team Qualifications Active Arizona Physical Therapist license CPR certification (AHA) Minimum 1 year of clinical experience (home health preferred) Strong clinical judgment and time management Reliable transportation, valid driver’s license, and insurance Strong communication and a patient-first approach What You’ll Gain Flexible, independent schedule Competitive per-visit compensation with strong earning potential Consistent patient volume to support productivity A meaningful role helping patients remain safe at home #J-18808-Ljbffr
