Key Responsibilities\n

• Conduct thorough ongoing assessments to evaluate and understand the individual needs of patients, guiding care decisions with evidence-based approaches.

• Facilitate effective communication between patients, families, and multidisciplinary team members to ensure comprehensive care and support.

• Develop and provide a schedule for appointments, home visits, and observations, maintaining an effective workflow that prioritizes client needs.

• Coordinate home visits, incorporating in-home observations, chart reviews, and data analysis to enhance patient outcomes.

• Ensure that the implementation of the treatment program plan is adhered to and evaluate its effectiveness regularly.

• Participate actively in Interdisciplinary Team Meetings to share insights and strategies focused on individual care progress.

• Document and complete detailed monthly and quarterly progress notes to track and communicate developments in each patient's journey.
\n
• Organize and deliver training sessions for team members on program implementation and documentation practices to foster competencies within the staff.
\n
• Provide ongoing follow-up, including interviews, home visits, and observations, to ensure sustained support for the patient's ongoing needs.

• Maintain accurate and confidential records to ensure that essential information is accessible to facilitate care continuity.

• Act as a liaison between the community and mental health resources, promoting public awareness and support for developmental goals.

• Collaborate to create innovative training materials and procedures to enhance overall program effectiveness.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field.

• An active and unrestricted license as a Professional Counselor (LPC) in the state of Georgia.

• Expertise in specific therapeutic areas such as family therapy, couples therapy, substance misuse counseling, or trauma-informed care strongly preferred.
